Liberty health insurance claim form can be downloaded from the website of the company or can be downloaded from here directly.


  • The claim form should be duly filled and sent to the insurance company for processing the claim on a cashless basis.
  • The cashless claim settlement would happen only when the claim form is duly filled and relevant documents are attached with it before sending to the insurance company.
  • The claim authorization would be given by the TPA in consultation with the insurance company.
